We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Co-Ordinator will be responsible for ensuring that the practice meets its QOF targets through diligent patient monitoring and administration. This role involves managing patient invitations for chronic disease and drug monitoring, supporting research and national health campaigns, and performing regular checks to ensure accurate coding and monitoring. The post holder will also handle various administrative tasks for the GP's and nurses, , liaising with nursing homes, and managing medical student rotas'. Additionally, act as the first contact for outside health agencies and frontline NHS staff
key responsibilities
A. QOF, Recalls & Long-Term Condition Monitoring
Run population reporting searches and follow-up actions for QOF.
Manage monthly recalls for diabetes, asthma, and NDH patients.
Code and track average blood pressure readings in EMIS.
Organise learning disability reviews and send pre-appointment questionnaires.
Monitor diabetic patient follow-ups and blood test reminders.
Attend and minute quarterly diabetic clinical meetings.
Maintain diabetic follow-up lists and ensure actions are completed.
Send weekly asthma questionnaires to relevant patients.
B. Clinical & Patient Administration
Manage incoming/outgoing emails and generic inbox.
Handle Accurx communications and Docman/EMIS tasks.
Schedule GP medication review and care home ward round clinics.
Set and maintain monthly ward round rotas and reminders.
Build and maintain nurse templates (minor ops, coils/implants, travel).
Track baby immunisations with Southern Health.
Support GP rotas for medical students (logins, timetables).
Log and coordinate significant events and documentation.
Update Covid and flu housebound data in Outcomes 4 Health.
Process private/non-NHS correspondence requests (e.g., DVLA, HGV).
Manage IGPR + SAR medical record requests.
C. Safeguarding admin Support
Identify and escalate safeguarding concerns (Section 17/47).
Collaborate to code and follow up safeguarding documentation.
D. Clinic & Service Coordination
Book and coordinate appointments for:
Coil and implant clinics
Minor operations
Travel risk assessments
Spectra Diabetic Eye Screening
Liaise with external services (e.g., district nurse for bloods/wound care).
E. Medication & Prescriptions Support
Coordinate GP medication reviews.
Manage queries regarding prescription exemption certificates.
F. Facilities & Governance
Conduct water safety checks.
Ensure compliance with data protection and information governance.
Contribute to significant event reviews and quality improvements.
Secondary Responsibilities
Participate in audits as requested.
Provide reception cover during staff meetings.
Book appointments to support other teams.
Assist medical secretaries with urgent referrals during absences.
Perform other admin tasks as needed based on workload and staffing.
Skills
Previous office experience is essential for managing administrative tasks efficiently.
Excellent communication skills, both verbal and written, to interact effectively with patients and staff
Strong organisational abilities with attention to detail to manage multiple tasks simultaneously.
Empathy and compassion for patients' needs, ensuring a supportive environment.
Proficiency in using Microsoft office, Emis Webb and Docman clinical systems for documentation and communication purposes.
