The Nurse in Charge Care Home Advanced Practitioners (CHAP's) are members of the team who have received extra training to enable them to assist the nurse in a wide range of clinical practice. CHAP's are not qualified Nurses and must not be treated as such or given the full responsibility of a qualified nurse's role.
SKILLS, KNOWLEDGE & QUALIFICATIONS
Required: o Complete the CHAPS Training and further training as and when required. o NVQ HSC Level 2 or QCF HSC Award 2 (or higher) in Health and Social Care (or equivalent). o Ability to communicate effectively at all levels. o Genuine interest in working with the relevant Service User group. o Satisfactory Police Check and check against the SOVA List. Desired: o Team player. o Experience and qualification in care for relevant Service User category. o Previous supervisory experience.
MAIN RESPONSIBILITIES
Care: 1. To liaise and work with the nurse in charge in the provision of quality care for all Service Users coming into and living in the Care Home. To ensure the provision of the highest level of care and support to Service Users, Relatives and others. To promote a person-centered care philosophy throughout all areas of the service.
2. Support training and supervision of junior and new Staff Members in all aspects of their work in the Care Home, under the supervision of senior Staff Members.
3. Ensure that all Staff Members contribute, to the best of their ability, to the efficient running of the Care Home and the creation of an atmosphere conducive to the best interests of the Service Users. Take the lead, in the absence of the nurse on duty, ensuring the floor is running efficiently and safely ensuring to seek advice by another nurse in the building for tasks out of the Care Home Advanced Practitioners remit.
4. 5. Report any ill health amongst Service Users and make requests for GP/Professional visits where necessary. Assess social and health care needs of new Service Users and maintain on-going assessment and review of all other Service Users.
6. Maintain accurate records and ensure that each Service User within the Care Home has an individualised Care Plan. Service User Care Plans to be reviewed on a monthly basis, or more frequently as and when required, in accordance with Company policy.
7. Carry out regular checks on Service Users at intervals determined by the Nurse in charge. Ensure a clear and concise handover report is given to all relevant Staff Members at the end of each shift.
8. Administer prescribed medicines and document the same in accordance with the Company's procedure and guidelines.
9. Practice maximum integrity in all dealings with Service Users' personal and financial affairs and avoid abuse of the privileged relationship which exists with Service Users. Clinical/Quality
10. Carry out blood sugar monitoring in line with service user care plan and report when the result is not within the range deemed as acceptable for the Service User.
11. Carryout Venepuncture as requested by the nurse on behalf of the GP
12. Carryout simple wound dressings as requested by the nurse and complete appropriate documentation.
13. Carryout NEWs in line with policy.
14. Care for catheters and Stomas in line with care plan, this does not include catheterisation.
15. Check and monitor syringe drivers in line with the care plan as requested by the nurse.
16. Lead and guide the staff team to deliver care and support of the highest quality.
17. Manage all aspects of medication in the Care Home as per Policies and Procedures and in line with your competencies.
18. Support and assist the Registered Nurse in the audit process for the Unit for all areas and ensure timescales are met.
19. Assist the Registered Nurse to implement the incident and accident reporting process, contribute to analysis and remedial actions.
20. Ensure a clear and concise handover report is implemented at the change of shift and complete an allocation of duties for care staff.

23. Report any changes in Service Users physical or emotional condition to the Home Manager or Nurse in Charge as applicable.
24. Provide comfort and company on a one to one basis for Service Users who are confined to their room for any reason.

29. Organise and deliver induction programmes for all new care staff. Supervise and support them in all aspects of their role.
30. Maintain and improve professional knowledge and competence, using evidence based best practice and attend training.

32. Report immediately to the Home Manager or Nurse in Charge any illness of an infectious nature or accident incurred by a Service User, colleague, self or another.
33. Understand, and ensure the implementation of the Care Centers Health & Safety policy and Emergency and Fire procedures.
34. Report to the Home Manager or the Handyperson any faulty appliances, damaged furniture or equipment or any potential hazard.
35. Promote safe working practice in the Care Home.
36. Ensure that all information of confidential nature gained in the course of duty is not divulged to third parties.
37. Notify the Home Manager or Nurse in Charge as soon as possible of your inability to report for duty and on your return to work from all periods of absence.
38. Promote and ensure the good reputation of the Care Home.
39. Ensure the security of the Care Home is maintained at all times.
40. Adhere to all Company policies and procedures within the defined timescales.
41. Ensure all equipment is clean and well maintained.
42. Carry out any other tasks that may be reasonably assigned to you
