Hours: Nights 33 Hours per week available, working weekends on rota (Shift Times 19:30 to 07:30)

ABOUT THE
CARE HOME ASSISTANT PRACTITIONER
ROLE
Within the Care Home Assistant Practitioner role, you can really make a difference to our residents' lives. Your responsibilities will include:
Responsible for the provisions of exceptional person-centred care, collaborating with the nursing team within the care home.
Delivery of excellent standards of care, to enhance the lives and emotional wellbeing of each resident.
Acting as an ambassador, by role modelling best practice.
Supporting and collaborating directly with the nursing team within the home to ensure the smooth running of shifts, including accurate and legible completion of documentation.
ABOUT YOU
Our fantastic staff are at the heart of our homes & provide vital day-to-day support to our residents, so we would love to hear from those who really do want to make a difference to people's lives.
A caring, compassionate, and competent team member, who works well as part of a team and can lead by examples of best practice. Demonstrating a passion and commitment for working within the care home.
Has the ability to multi-task in a high pressure and demanding environment with care and compassion.
Outstanding communication and interpersonal skills are required.
Positive work ethics, with the passion for ensuring the delivery of exceptional care standards are kept at the highest priority.
To engage professionally with both residents, families, team members, and stakeholders respectfully and with compassion.
Excellent time management skills with the ability to be flexible to change to meet the requirements of the home.
A keenness to develop and motivate others within the team by creating a positive culture within the home.
