Job Description

This is an exciting opportunity for an experienced Nursing Associate to work as part of our friendly and dynamic multi-disciplinary care team.



You must be an enthusiastic individual with excellent Communication, organisational skills and motivation in your approach to care delivery to ensure we maintain a high-quality service for our patients in the changing environment of community services.

Care Home practitioners (CHP) work as part of the wider health and social care team and have direct contact with patients within the Residential home setting, providing high quality and compassionate care.

CHPs work at a level above that of Healthcare Support Workers and have a more in depth understanding regarding patient care. CHPs provide training and support to care homes in line with current policies and guidelines.

CHPs work closely with their respective GP surgery linked to the residential homes and other Health and social services.



The post holder carries continuing responsibility for ensuring adequate training and guidance is carried out within the Residential care setting.



General clinical aspects of the role may include:



Undertaking tasks delegated by a Registered Practitioner



Assisting in patient assessment



Referrals to the Registered Practitioner and other MDT's



Physiological measurements



Risk Management, infection control, health and safety



Higher clinical skills such as catheterisation, wound care and phlebotomy, continence assessments within the residential homes.



Non-clinical aspects of the role include:



Advance care planning/ Admission avoidance



Supervision and teaching with Care Home staff



Personal development



Team working



Communication



Person-centred care and wellbeing

Quality and improvement.



Working closely with Care home staff and GP Surgeries



Supporting Community Matrons and GPs during Residential ward rounds and Multi-disciplinary working.



The post holder will work as an autonomous practitioner within the Community Nursing Team, supporting the delivery of care and supporting patients in residential homes, collaborating with system partners and as part of the Primary Care Networks and Integrated Neighbourhood Teams as part of the out-of-hospital model of care.
