Job Description

Blue Sky Enabling are a specialist provider of social care support services based in Trowbridge working with adults with LD/ASD and complex behaviour. We are recruiting for an experienced, Care Manager who must have experience in working in services for people with learning disabilities, autism, complex needs and challenging behaviour. They will manage 4 service users who each live in their own independent home in the community.

We seek someone who is willing to work collaboratively with the rest of the management team to bring dedication and energy to the role, someone to inspire passion, invite co-operation, and who will engage the staff and service users with the organisational culture. You must hold relevant qualifications and training to at least QCF/NVQ Level 3 in Health & Social Care. You must be able to evidence at least 1 year experience managing services, ideally for people with LD/ASD and challenging behaviour. You must be able to demonstrate collaborative working with multi-agency teams and families and creative problem solving through a person-centred approach to issues.

Your actual salary will be dependant upon the Level of qualification and years of relevant experience.

You will:

- Work collaboratively as part of the management team

- Oversee the provision of quality person-centred services to a small portfolio of service users with LD/ ASD/MH and challenging behaviour

- Support the recruitment, development, supervision and monitoring of staff team members

- Contribute to a positive working environment

- Work with service users, their families, professionals and staff to create, review and update support plans, PBSPs, Risk Assessments

- Develop a positive relationship with the service users you manage, carrying out regular visits, conducting audits and observations

- Contribute to weekly care management meetings, reporting on the progress of your service users

- Your hours are generally 40 hours a week Monday-Friday with a 30min lunch break, plus contribution to the on-call rota, but you may be required to work additional hours or at weekends if the work demands it.

- A blended work schedule allowing you to work from home Monday and Friday and in the office/on site.
