Care Navigator
Job Description
Are you looking for an exciting opportunity to join a forward thinking team under the leadership of a friendly, dynamic multidisciplinary partnership? Nant Dowlais Health Centre opened on 1 July 2020, serving a 10,000 patient list in West Cwmbran. We are seeking an additional Care Navigator to expand our team.

5 weeks of annual leave
The successful candidate will also be required to cover annual leave and sickness at short notice when needed.
