Job Description

As a Care Navigator, you will be the vital link between patients and the healthcare system. Your role involves helping patients access the right services, supporting their journey through care pathways, and reducing the workload on GPs by managing non-clinical queries. You will work closely with healthcare teams to improve patient experience, promote self-care, and ensure timely, coordinated care.

Main duties of the job
Engage with patients to understand their needs and direct them to appropriate healthcare and community services.
Support patients in navigating the healthcare system, including booking appointments and making referrals.
Help reduce GP workload by managing non-clinical patient queries and signposting.
Maintain accurate records of patient interactions and referrals.
Work collaboratively with the healthcare team to improve patient care pathways.
