The Care Navigator plays a crucial role in supporting patients through their healthcare journeys. Responsibilities
Serve as the primary point of contact for patients when accessing the service.
Main responsibilities will be call handling and managing the front reception desk.
Maintain accurate records of patient interactions and progress within the office system.
Collaborate with healthcare providers, social workers, and community resources to facilitate any patient queries.
Requirements
Previous experience in an office environment is preferred.
Strong interpersonal skills with the ability to communicate effectively with diverse populations.
Excellent organisational skills and attention to detail to manage multiple tasks efficiently.
Empathy and a genuine desire to help others navigate their healthcare challenges.
Proficiency in using office software and electronic health record systems is advantageous.
