Job Summary


The Care Navigator plays a crucial role in supporting individuals through their healthcare journey. This position involves guiding patients and their families in accessing appropriate services, resources, and support systems. The Care Navigator will work collaboratively with healthcare providers to ensure that patients receive comprehensive care tailored to their unique needs. Strong organisational skills and an understanding of office procedures are essential for success in this role.

Responsibilities



Assist patients in navigating the healthcare system by providing information on available services and resources. Coordinate care plans in collaboration with healthcare professionals, ensuring that all aspects of patient care are addressed. Maintain accurate records of patient interactions and updates to care plans. Facilitate communication between patients, families, and healthcare providers to ensure continuity of care. Provide education to patients regarding their health conditions and available treatment options. Support patients in understanding their rights and responsibilities within the healthcare system. Organise follow-up appointments and ensure that patients adhere to their care plans. Participate in team meetings to discuss patient progress and identify areas for improvement.

Qualifications



Proven experience in an office environment, demonstrating strong organisational skills. Excellent communication skills, both verbal and written, with the ability to convey complex information clearly. Empathy and a genuine desire to help others navigate their healthcare needs. Ability to work independently as well as part of a multidisciplinary team. Familiarity with health-related terminology is advantageous but not essential. Proficient in using office software and managing electronic health records.

Job Type: Full-time

Pay: From 12.21 per hour

Expected hours: 37.5 per week
