THC Care have an exciting opportunity for an experienced Care Plan Assessor to join part of the management team. The successful candidate will be responsible for updating, reviewing, and assessing all service user's needs including personal care and medication to enable them to build a person-centered care plan.
The Assessor will also be responsible for communicating any changes or updates effectively to all care workers and service users.
The Role
12.20 per hour
48.00 Hour contract
Working pattern to be discussed can be flexible with days and times.
28 days holiday entitlement (Pro rata)
Flexibility is essential.
level 3 Health and Social care qualification is preferred but not essential. THC Care are willing to support career progression and help support to gain Health and Social care Level 3.
To produce accurate risk assessments all aspects of care and client environment.
Ability to build person centred care plans.
Essential Skills
Excellent verbal and written communication skills are essential as well as a high standard of literacy and numeracy skills.
Ability to work on their own but also part of a team. Being able to use your own initiative is essential for this role and support others.
Candidates should be conversant with standard office software packages ie. word, excel, outlook and previous experience using CM2000 is preferred.
Adhere to all quality requirements to ensure to an exceptional service is achieved.
Have the ability to work in fast-paced environment.
Carry out all other duties relevant to the role as directed by the Registered Manager.

Minimum Health and Social care Level 2
Full UK Drivers License
DBS Check will be required for successful candidates.
