Purpose of the Role
The role involves hands-on care and support, with a negotiable amount of time allocated for conducting other duties outlined below.
The Care Practitioner is responsible for ensuring high-quality domiciliary care services in compliance with CIW regulations. This includes conducting care package assessments and reviews, supporting carers, and ensuring service users receive person-centred care. The role also involves rota management, shift cover, staff development, and maintaining a proactive and innovative approach to problem-solving.
Key Responsibilities
Care Package Management & Assessments
Conduct assessments and reviews of care packages to ensure they meet service users' needs.
Liaise with social workers, community resource teams (CRT), and other professionals for holistic care planning.
Support individuals to live meaningful lives in a way that matters to them.
Staff Support & Development
Provide guidance and support to carers, ensuring high standards of care delivery.
Encourage a positive working environment, fostering staff motivation and engagement.
Assist with rota management and compiling staff runs to ensure efficient service delivery.
Operational Duties
Conduct spot checks to ensure quality and compliance with care standards.
Cover shifts as required to maintain service continuity.
Participate in the on-call rota to provide emergency support.
Assist with IT systems and be willing to learn new digital tools for care management.
Problem-Solving & Innovation
Take a proactive approach to addressing challenges and improving care processes.
Offer solutions to operational and care-related issues to enhance service quality.
Requirements & Qualifications
Must be registered with Social Care Wales (or eligible for registration).
Experience in domiciliary care or a similar care setting.
Strong understanding of CIW regulations and person-centred care principles.
Proficient in using IT systems for care management and reporting.
Ability to work independently and as part of a team.
Positive attitude, with a focus on supporting and encouraging staff.
Full UK driving licence with access to a vehicle.
Willingness to participate in an on-call rota.
Ability to think innovatively and provide solutions to challenges.
Working Conditions
The role involves a combination of admin/hands-off time and hands-on care work.
Participation in the on-call rota as required.
Travel within Gwynedd and Anglesey may be required.
