Summary of Position:

The Carers Assessment office is responsible for delivering high quality Assessments, support plans and reviews in Line with the Care Act 2014. The role promotes health and wellbeing for carers ensures they are informed of their rights and facilitated access to appropriate support and services

Responsibilities



Primary Tasks

Responding to requests for carers assessments in a timely and professional manner.

To support carers through the assessment process. This will include home visits, telephone community outreach

Develop and review support plans tailored to individual needs

Apply a strength based and trauma informed approach to assessments.

Maintain accurate records using Mosaic and Charity log systems (training will be provided)

To use an asset-based approach considering all services available to carers.

Identify carers support needs and connect them with statutory, community and voluntary sector services

Collaborate with Health and Social Care professionals for complex cases.

Manage and Priortise caseloads effectively

Ensure Carer registrations comply with Harrow Carers policies.

Produce monitoring reports as requested by the Head of Adult Serivces

Raise safeguarding concerns and DOLS concerns where appropriate.

Secondary Tasks

Signpost and refer carers to external services either universal or statutory

To support colleagues in other areas of the organisation as when required

To undertake additional responsibilities as designated by Harrow Carers

To comply with General Data Protection Regulations relating to personal information of individuals of the public

To comply with all relevant policies, procedures, and guidelines, including those relating to Safeguarding, Equal Opportunities, Health and Safety, Confidentiality and Dignity at Work.

Attend regular team meetings, case reviews and supervisions when required.

It is a required of this role to undertake clinical supervision.

Participate in Annual Appraisals .

Person Specification



Qulaifications



Level 3 In health and Social Care or relevant qualifications or equivalent experience.

Experience and Knowledge



Experience of providing front line, holistic support to vulnerable people.

Proven ability to carry out assessments reviews and Support planning.

Background in a health and social care setting

Experience of working with external professionals and organisations

Good working understanding of supporting unpaid carers and the challenges they face

Knowledge and understanding of welfare benefits systems for unpaid carers & Care Act 2014

Understanding of Safeguarding vulnerable adults and deprivation of liberty procedures

Experience of Deprivation of Liberty Procedures.

Knowledge of Mental Health Act 2005.

Good knowledge of Universal Care Plans.

Understanding and Commitment to Equality of Opportunity and working practices

Experience of using Charity log (desirable)

Experience of using Mosaic systems (desirable)

Experience of completing monitoring and evaluation reports.
