Caseload Manager Band 6
An exciting opportunity has arisen for motivated individual to work within the Specialist Community Nursing Division as a Community Nursing Caseload Manager, within our team at St Catherine's Health Centre.
This post will be responsible for managing planned programmes of care for patients in domiciliary and clinic settings.
Hours per week are 22.5, shift hours to be discussed at interview.
A main part of the role of a Caseload Manager is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.
You will need to be able to work autonomously and as part of a team and possess excellent time management, organisational and communication skills. You will be innovative, flexible and prepared to adapt to the changing needs of the Trust as they arise.
