The Community Assessment Team (CAT) is based at the Emergency Departments across Gloucester Royal Hospital and Cheltenham General Hospital. The teams purpose is to provide a rapid assessment of patients to prevent admission to the acute trust. We work between the hours of 8am-8pm, Monday to Friday, and 8am-6pm weekends and bank holidays.
The post holder will be responsible for assessing, planning, implementing and evaluating high-quality, holistic patient care, within the Emergency Department.
For new starters, there will be a 6-8 week supernumerary induction period and you will be allocated a mentor to support.
To assess a cohort of patients within the Emergency Department (ED) from presentation through to discharge.
Through training and development educate GHNHSFT and colleagues about available discharge pathways and community services.
Working with colleagues, to risk assesses suitability for discharge and ensure that arrangements are appropriate and timely.
To communicate information upon discharge to multidisciplinary teams, external agencies, patients and carers.
To work with community services, ie CATU, SPCA, GHC Rapid Response Team (RR).
Lead and support the wider team on those patients who require a more in depth Occupational Therapy or Physiotherapy assessment.
Line management responsibility as required.
