We are seeking a dedicated and compassionate Community Nurse Specialist to join our team at G&P Healthcare. The ideal candidate will play a vital role in delivering high-quality nursing care to clients within the community. This position requires strong leadership skills, the ability to develop and implement care plans, and excellent time management abilities.
Responsibilities
Working with our current community nurse specialist, to train our staff to a high standard in all aspects of complex care with children and/or adults in their home environment, Including Airway Management, Ventilation, Tracheostomy, PEG and Seizure management.
Develop, implement, and evaluate individualised care plans in collaboration with patients and their families.
Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes.
Educate patients and their families about health management, disease prevention, and medication adherence.
Maintain accurate and up-to-date patient records in accordance with regulatory standards.
Communicate effectively with multidisciplinary teams to coordinate patient care and services.
Demonstrate strong leadership by mentoring junior staff and supporting their professional development.
Manage time efficiently to balance patient visits, documentation, and administrative tasks.
Qualifications
Registered Nurse (RN) qualification with valid nursing registration.
Minimum of 2 Years Post experience.
Experience with Ventilation and Tracheostomy
Proven experience in community nursing or similar healthcare settings is preferred but not essential.
Strong leadership skills with the ability to inspire and guide others.
Excellent time management skills to prioritise tasks effectively.
Exceptional communication skills, both verbal and written, to engage with patients, families, and healthcare professionals.
Ability to work independently while also being a collaborative team player.
