About the role:
This is an exciting opportunity to shape and deliver innovative approaches to supporting young people with a disability as they leave care. This role will be part of a 3 year funded project working alongside our Staying Close Staying Connected Service, our leaving care service funded by the Department of Education. For children in care, turning 18 can mean finding themselves facing a cliff-edge of support. This service is truly leading the way nationally in terms of leaving care provision and we want to ensure all young people can benefit.
This is one of two posts supporting delivery of this project, the other is a Housing Innovations and Partnerships Manager. We will consider applications for individuals interested in undertaking both roles.
You will work with Break's Development Manager, Co-Production & Aspiration Lead, and young people to:
This is an exciting role which requires an innovative approach, the ability to build and maintain effective external partnerships and an ability to turn ideas into impact.
You will have experience of partnership working with local authorities, , health services, voluntary organisations, or community partners. As well as the knowledge of relevant legislation, policy, and best practice relating to disability rights, care leavers and transitions.
