This post will offer the successful candidate an exciting opportunity to become part of a multi-agency service for children with Complex Needs Team, offering a unique opportunity to work within a co-located multi-agency setting fostering partnership and collaboration between children, young people, their families and other professionals. We work very closely with the Learning Disability Child Adolescents Mental Health Service and the Children's Intensive Behaviour Support Service and there are opportunities for joint working.
We are looking for an innovative and motivated person who has a Registered Nurse Qualification in Learning Disabilities with experience of working in a multi-agency environment to join our Children Learning Disabilities Nursing Team. The successful candidate will work with children and young people who have learning disabilities, complex health needs and/or challenging behavior. Therefore you will need good observational, assessment and analytical skills and have previous experience of working in community or residential settings.
The applicant should have working knowledge and experience of using evidence based assessment tools and an interest in continence, health initiatives and positive behavior support.
CLDNT offers specialist placement for Undergraduate LD Nurses. There will be an opportunity for the successful candidate to supervise students on placement.
Key Responsibilities - Professional/Clinical
1. To provide a service for children and young people from 0 - 18 years with significant learning disabilities.
2. Works with an allocated defined caseload, working independently in a responsible, innovative and flexible manner.
3. To assess the heath needs of children and young people with significant learning disabilities, plan, monitor, evaluate and review care packages.
4. To assess and formulate individual positive behavioural support plans for children and young people whose behaviour challenges
5. To promote inclusion and raise awareness of health care needs and facilitate access to primary/health care services.
6. To comply with policies and guidelines as directed within the Central Manchester University Hospitals NHS Foundation Trust.
