Job Description

The Integrated Care Co-ordination for Urgent Care Team (ICC) is offering an exciting opportunity to join the team as the Clinical and Operational Manager based in Stoke on Trent.



You will lead the clinical and operational service delivery of the ICC, providing expert clinical knowledge and Operational Management including providing evidence-based care and interventions and ensuring patients presenting with an urgent care need are provided with the most appropriate advice, care and support at the right time, in the right place by the right service.



You will offer you expert advice, support and supervision to colleagues to shape and deliver the best in quality care and provide nursing and health care professional leadership with line management of staff with professional registrations.



Working closely with the Clinical Team Leader, Associate Director of Urgent Care, Head of Service, Operations Director you will develop and deliver safe and effective services, in accordance with key national and internal targets and objectives including clinical standards, service priorities and key performance indicators.



The service operates from 8am to 8pm. This role is predominately Monday-Friday 9am-5pm with an expectation of some flexibility to meet service needs



As the Clinical and Operational Manager, you will lead and manage the ICC in accordance with Trust values, in the delivery of effective, efficient and high-quality services for Service Users and their Families / Carers.



As part of this role, you will be expected to:



Lead the development, implementation and evaluation of clinical processes within the team and contribute to the development of clinical urgent care pathways Ensure internal and external partners have a responsive point of contact to support the needs of patients presenting to the ICC with an urgent care need Ensure the maintenance and monitoring of clinical service standards. Participate towards the Care Group's formulation of strategic and operational plans for the directorate, which take into account local, regional and national drivers. Develop effective working relationships with partner agencies to improve outcomes for service users. Analyse data and information relating to service performance to inform future service developments. Including regular evaluation of current trends and initiate changes in the light of these findings. Lead on local service development and redesign and effectively manage service change across the workforce. Establish effective clinical governance systems and processes e.g. facilitating clinical audits. Implement recommendations from clinical audits with support from the Clinical Care Director, Clinical Lead and Professional Leads.
