We're looking for an
Associate Case Manager
who brings
expertise
and
compassion
in equal measure. If you have a background in
Occupational Therapy

Physiotherapy
,
Psychology
, or
Nursing

rehabilitation
journey, ensuring they feel
supported

What You'll Be Doing
Assess client needs and develop tailored rehabilitation plans
Coordinate and oversee therapy and support services
Work collaboratively with clients, families, and healthcare professionals
Monitor progress and adjust plans as needed to promote independence
Provide expert advice and advocacy to ensure clients receive the right support
Maintain accurate records and documentation in line with professional standards
About You
A qualified professional with a background in Occupational Therapy, Physiotherapy, Psychology, or Nursing
Experience working in neurorehabilitation, case management, or complex care (preferred but not essential)
Excellent communication and interpersonal skills, with a compassionate and client-centred approach
Strong organisational skills and the ability to manage a diverse caseload
A proactive problem-solver who can work independently and as part of a team
Commitment to professional development and delivering high-quality care
