What You'll Do
As an Associate Case Manager, you'll coordinate the many threads of rehabilitation so that clients and families feel supported, heard, and empowered. Your role will include:
Client & Family Support
- walk alongside families through their emotional journey, problem-solve practical barriers, and ensure their voices are central among the professional networks.
Care Coordination
- set up and oversee care packages (agency or private), source placements where needed, and work with statutory services to secure funding and provision.
Therapy & Services
- identify, instruct, and coordinate independent therapists and specialists (e.g. psychology, orthotics), ensuring quality and measurable outcomes.
Education & Vocational Support
- liaise with schools, SENCOs, and local authorities to secure EHCPs; support safe return-to-work planning with employers.
Home & Equipment
- arrange adaptations and specialist equipment, collaborating with therapists, architects, and builders to maximise independence.
Community Access
- promote inclusion in leisure, social, and vocational activities, from accessible vehicles to hydrotherapy and riding for the disabled.
Legal & Reporting
- maintain accurate records, update legal/insurer teams, and produce clear, timely, high-quality reports.

Who We're Looking For
You'll be:
An experienced clinician (3+ years post-qualification)
Offering trauma-informed and culturally sensitive support
Able to carry out home visits
Applying strong clinical reasoning
Providing passionate about genuine person-centred practice.
willing to embrace the autonomy and flexibility of case management.
Eligible professionals include:
Social Workers (SWE registered)
Nurses (NMC registered - Adult, Child, or Learning Disability)
Occupational Therapists (HCPC registered)
Physiotherapists (HCPC registered)
Speech & Language Therapists (HCPC registered)
Location & Flexibility
Remote/home-based role
Travel required for client visits (currently South-East, but planning to expand)
Flexible hours; most Associates offer 2-3 days per week input
