Silverdale Practice is a two site GP Surgery providing health services to 13,650 patients across Burgess Hill. We are a dynamic and forward thinking Practice that embraces technology to provide safe, efficient and effective services for our patients. We receive a great deal of information into the practice each day and throughout the day. This comes by a variety of routes, both electronic and paper-based. The correct handling of this information is vital to ensure safe patient care and the efficient running of the practice. The information requires an excellent eye for detail and the role requires the information to be recorded efficiently, safely, accurately and diligently.
As an important member of the The Care Coordinator Team the successful applicant with work to protocols underpinned by robust clinical governance and regular audits to ensure accuracy and that correct procedures are followed with regards to the information.
The Team consists of a Team Lead and 1 other Care Coordinator and a Clinical Coder/ Workflow Administrator. The Team works together supporting frail, elderly and vulnerable patients to maintain their independence and avoid unnecessary Hospital admissions or admissions to care homes. Working closely with the Practice primary care teams, to ensure patients receive a joined-up service and the most appropriate support.

1. SCOPE AND PURPOSE OF THE ROLE
It is the responsibility of each Clinical Coder to screen all incoming medical letters, entering key data and selecting and directing appropriate information for GPs attention adhering to the practice policy. The role will involve electronically filing and summarising patients' medical notes. This role can be achieved either working remotely or in practice.
The roles and responsibilities may change according to the business requirements.
2. PRIMARY DUTIES & AREAS OF RESPONSIBILITY
Review all incoming correspondence filing the document and entering pertinent information into the patient's
Electronic medical record in accordance with the Coding Protocol with attention to QOF.
Highlight clinician action and workflow to appropriate clinician or team as per Workflow Protocol.
Organise the medical record of patient whom you are coding, to link problem titles.
Checking incoming medical records and summarising onto clinical system.
Recognise own knowledge limits and seek advice from clinical or administrative colleagues as appropriate.
To carry out administrative related tasks as delegated by Clinical Lead and Practice Manager.
Other tasks

JOB DESCRIPTION AGREEMENT

This description will be open to regular review and may be amended to take into account development within the Practice. It is important that all members of staff are prepared to take on additional duties or relinquish existing duties in order to maintain efficient running of the practice. The practice reserves the right to re distribute duties and functions amongst members of staff from time to time and requests for such changes shall not be reasonably refused.
This is not exhaustive, other tasks and responsibilities may be required after discussion with line manager.
Minimum of 3 years' experience in Healthcare Setting. Must have excellent IT skills including use of WORD, PDF and Excel.
