We are seeking a new Clinical Lead for our Learning Disability & Autism service based at the Bristol Royal Infirmary (BRI) Hospital, covering all of University Hospitals Bristol and Weston (UHBW) sites. You will be a part of the wider Acute LD&A Liaison leadership team, ensuring a consistent and joined up service across all hospital sites in Bristol, North Somerset and South Gloucestershire.
Your primary purpose will be to lead the planning, provision and improvement of the service by ensuring high quality clinical practice to individuals including clear record keeping and case load management systems. You will supervise junior staff and students. You will also lead and coordinate a number of projects that will span the Hospital as well with your wider acute liaison colleagues in North Bristol Trust.
As part of the Adult Learning Disability Health Service you will also work closely with your colleagues in the community to ensure continuity of care from admission planning to post discharge follow up.
Lead on operational management, supervision and appraisal of practitioners within the Acute Liaison Service including clinical records systems and caseload management in order to ensure contractual performance requirements are met.
Demonstrate leadership behaviours including (but not exclusive to) autonomous practice, self-reflection and awareness, self-confidence, the ability to facilitate self-awareness in others, resilience and determination.
To be a part of the Acute Liaison Service leadership team, providing a joined-up liaison service across Bristol, North Somerset and South Gloucestershire. Including co-ordinating projects, training, work force planning, attending regular leadership meetings and providing leadership cover when required.
Liaise with acute hospital staff and key acute hospital services (i.e. Safeguarding, Discharge planning, Matrons) in order to ensure joined up agreed systems of working to enhance the patient experience.
To accept referrals and assess individuals needs in order to provide reasonable adjustments to facilitate access for people who have a learning disability and / or autistic people and are accessing the Hospital services.
You will be responsible to the Clinical and Operational Lead (Sirona Adult Health Learning Disability Service) for the planning and delivery of high quality, safe and cost effective provision of the Learning Disability & Autism Liaison Services at the Hospital.
