Job Purpose



The Clinical Lead is responsible for ensuring the safe, effective, and high-quality delivery of clinical care across all homecare and supported living services. This role oversees clinical governance, supports risk management, ensures compliance with CQC standards, and provides expert clinical leadership to staff delivering care to adults and/or children with complex needs.

The Clinical Lead will champion best practice, lead on competency assessments, oversee care planning, and act as the clinical point of escalation for service users, families, commissioners, and multidisciplinary teams.

Key Responsibilities



A. Clinical Leadership & Oversight



Provide day-to-day clinical leadership across all assigned homecare packages and supported living sites. Ensure all clinical tasks (e.g., PEG feeds, tracheostomy care, epilepsy management, medication administration, wound care) are delivered safely and competently. Conduct clinical assessments and contribute to initial assessments, reviews, and complex case planning. Lead on clinical triage, incident review, and escalation of risks.

B. Care Planning & Clinical Governance



Develop, review, and quality-assure person-centred care plans, clinical risk assessments, and support plans. Ensure compliance with CQC Fundamental Standards and company policies. Review and analyse clinical audits, medication audits, MAR charts, and incident reports, implementing corrective actions. Oversee infection control compliance and safe medication management.

C. Staff Training, Competency & Supervision



Deliver clinical training to nurses, HCAs, and support workers based on service needs (e.g., suctioning, oxygen therapy, seizure management). Assess clinical competencies and sign staff off as safe-to-practice. Provide clinical supervision and reflective sessions to staff. Support recruitment by participating in clinical interviews and skills assessments.

D. Quality Assurance & Compliance



Ensure services are inspection-ready and meeting CQC standards at all times. Maintain accurate clinical documentation in line with legal, professional, and organisational requirements. Lead on safeguarding concerns, contributing to Section 42 enquiries where required. Monitor service user outcomes and ensure timely updates to care plans.

E. Multi-Agency Collaboration



Act as the clinical liaison with GPs, District Nurses, MDTs, commissioners, hospitals, social workers, community teams, and families. Support hospital discharges, step-down pathways, and complex package mobilisations. Attend MDT meetings, case reviews, and safeguarding meetings representing the service clinically.

F. Service Development



Support mobilisation of new packages and supported living placements. Lead clinical elements of tenders, quality submissions, and commissioner meetings. Identify areas for improvement and implement action plans to enhance quality and safety.

3. Person Specification



Qualifications



Registered Nurse (RGN/RMN/RNLD) with active NMC registration essential. Clinical supervision or mentorship qualification desirable. Additional specialist clinical training (e.g., tracheostomy, ventilation, PEG, epilepsy) is an advantage.

Experience



Minimum 3 years post-registration experience. Experience in homecare, community nursing, supported living, or complex care essential. Experience supervising staff and managing clinical risk. Confidence leading on safeguarding, clinical governance, and quality assurance.

Knowledge & Skills



Strong understanding of CQC standards and regulatory requirements. Skilled in clinical assessment, risk management, and care planning. Excellent communication skills with ability to liaise with MDTs and families. Ability to teach, mentor, and supervise staff. Confident with IT systems, audits, and clinical documentation.
