Job Description

The post holder will play a key role in the neuro oncology and skull base multidisciplinary teams to deliver high quality care to patients. The clinical nurse specialist will be an experienced, registered nurse.

They will have a role in the co-ordination and day to day management of brain and CNS tumour patients undergoing surgery to ensure a high standard of service provision. The post holder will provide an information resource, counselling, education and practical support for patients, their families and healthcare professionals.

They will work across organisational boundaries and with primary care to ensure a seamless service from diagnosis and throughout the surgical pathway and referring to another speciality treatment centre or discharging of patients.

The post holder will be required to foster good working relationships and communicate effectively with all members of the multidisciplinary team and service users to ensure the delivery of high standards of care.

The post holder will engage in discussions in multidisciplinary team meetings, where appropriate will present patients, acting as an advocate for all patients with a diagnosis of brain tumours and contributing to adult services where appropriate.

They will provide a highly developed specialist knowledge underpinned by theory and experience as per the NICE guidelines.

Act in accordance with the Nursing and Midwifery Council Code of Conduct.



Clinical Practice

Acts as a key worker for a clinical caseload, working to coordinate care and provide easy access for ongoing support for patients. Perform comprehensive assessment of patient needs, plan, implement and evaluate care delivery according to changing healthcare provision. Collect, collate, evaluate and report clinical information, maintaining accurate patient and nursing records. Act as an advocate for individual patients; involve patients and relatives in the planning and delivery of care as well as evaluating the effectiveness of interventions and treatment protocols. Communicating highly sensitive and complex information to patients and carers using excellent advanced communication skills and expertise. Support patients to access, identify, appraise and interpret complex technical information, to enable patients to determine their treatment plan and own future. To undertake Health Needs Assessments on all patients and generate an personalised care plan addressing their needs. Work alongside other health professional and medical colleagues in oncology, ENT, endocrine, palliative care, radiology and pathology to ensure continuity of care is optimised. To deliver nurse led clinics (both telephone and face to face) as part of the patient pathway which may involve MRI results, symptom management, wound assessment, per-operative advice and support.

When required attend (on MS Teams or Face-to-Face) the daily neurosurgical meeting to identify oncology and skull base referrals and check the Orion system for any additional urgent tumour referrals. Join the daily ward rounds to identify and support post-operative patients,

identify and escalate any clinical concerns and work towards timely and comprehensive discharge.

Maintain own knowledge, competence and skill level through regular clinical supervision, appraisal, professional development plan and appropriate training programmes.

To provide expert nursing advice to patients, their families and healthcare professionals on cancer care and treatment, liaising and referring on to other specialist services as appropriate. To provide support and education patients and their carers from initial diagnosis through active and palliative treatments and follow-up care. To ensure that all patients and families are aware of all treatment options, including clinical trials. To facilitate support and continuity of care for patients through joint clinics with the consultant, this may including Teenage and Young Adult clinics, Best Supportive Care clinic or providing histology results. Provide a link for patients and their families between hospital, community and voluntary services to ensure continuity of care. Demonstrate professionalism, politeness, courtesy and sensitivity in providing compassionate care for patients, visitors/relatives and colleagues.

Keep accurate patient records and documentation, ensure the patient pathway is co-ordinated and targets met.

Organise own specialist workload, working autonomously as needed including education, training and cover. Take responsibility for own learning and performance including participating in team meetings and clinical supervision. Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from good practice. Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information. To act as a positive role model for nurses and HCA in the hospital through the demonstration of excellent communication and clinical skills. Understand responsibility of self and others to the Trust regarding the

Freedom and Information Act.
