Are you a compassionate and experienced Registered Nurse looking to make a meaningful difference in your community? Prospect Hospice is recruiting a Community Clinical Nurse Specialist (CNS) to join our expert team providing exceptional palliative and end-of-life care at home and in care settings.

Hours: Full and part-time positions available

over 7 days (including Saturdays or Sundays on a rota)

What is the role?

As a Community Clinical Nurse Specialist, you'll play a key role in delivering personalised, dignified, and responsive care to individuals with life-limiting illnesses. You'll manage a complex caseload, collaborate with multidisciplinary teams, support families, and provide clinical leadership and mentoring within our community services.

Key Responsibilities:

Deliver expert palliative and end-of-life care in the community

Conduct holistic assessments and manage complex symptoms Support anticipatory care planning and decision-making Act as an advocate for patients and families Educate and influence other health and social care professionals Contribute to clinical governance, quality assurance, and professional development Participate in out-of-hours service provision

Do you have?

NMC-registered with minimum 5 years of post-registration experience Strong knowledge of palliative care and symptom control Excellent communication and clinical decision-making skills Full driving licence and access to a car, prepared to travel throughout our defined catchment area (Swindon, Marlborough, Pewsey, Wootton Bassett, Lechlade, Fairford and Shrivenham) Willingness to undertake advanced specialist training (e.g., PACE, prescribing)

It would also be beneficial if you have:

Experience in community nursing or hospice care Non-medical prescribing qualification Formal or informal teaching experience
