The Role
We're looking for a dynamic and proactive Clinical Pharmacist to join our expanding team. You'll play a key role in medicines optimisation, structured clinical medication reviews, and improving prescribing safety across our network.
Working closely with GPs, nurses, and fellow pharmacists, you'll contribute to enhanced patient outcomes, reduce health inequalities, and provide specialist medicines advice within a supportive and forward-thinking environment.
Key Responsibilities
Conduct structured clinical medication reviews (including care homes and complex polypharmacy cases)
Provide patient-facing support via clinics and telephone consultations
Perform medicines reconciliation following hospital discharge or care transitions
Advise on and implement medicines safety initiatives and QI projects
Contribute to PCN prescribing audits, MHRA alerts, formulary reviews, and national guidance implementation
Support education and training of the primary care team
Help improve population health and reduce inequalities through medicines optimisation strategies
About You
We're looking for someone who is:
A GPhC-registered pharmacist with a degree in Pharmacy
Ideally an Independent Prescriber or currently working towards qualification
Experience working as a clinical pharmacist.
Knowledgeable in pharmacy law, NHS guidance, and medicines safety initiatives
A great communicator with strong problem-solving skills and attention to detail
Confident working independently and collaboratively within a multi-disciplinary team
Desirable:
Previous experience in a primary care setting
