JOB SUMMARY The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists and technicians in general practice. The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for undertaking clinical medication reviews to include proactively managing patients with complex polypharmacy. The post holder will provide primary support to general practice staff with regard to prescription and medication queries and provide expertise in clinical medicines advice while addressing both public and social care needs of patients in the practice.
The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. The role is pivotal to improving the quality of care and operational efficiencies and safety to required motivation and passion to deliver excellent service within general practice. CORE RESPONSIBILITIES 1. Working within the practice-based team to undertake structured medication reviews (SMRs) by telephone or face to face, particularly in high-risk groups such as: - Frail elderly - Polypharmacy - Renal.Hepatic impairment - Substance misuse - Patients on high-risk medicines 2.
Improve prescribing practice through educational support for all prescribers within the practice 3. Identification of cohorts of patients at high risk from harm from medicines through pre-prepared practice IT systems. This may include risks that are patient related, medicine related or both. 4.
Analyse, interpret and present medicines data, in conjunction with the Medicines Management team, to highlight issues and risks to support decision making. 5. Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues 6. Manage patients and health care professionals medicine tasks/queries.
7. Work with primary care professionals and patients to implement NICE and other evidence-based guidelines 8. Work with the practice team to ensure the practice is compliant with CQC standards where medicines and patient monitoring are involved. 9.
