Your Role
As our Clinical Pharmacist, you'll have a pivotal role in improving patient outcomes and supporting safe, evidence-based prescribing. Working with Pharmacy Team and GP mentors, you will:
Lead
clinical medication reviews
, especially for patients with complex needs and long-term conditions (e.g., COPD, asthma).
Deliver
patient-facing clinics and telephone support
, providing expert medicines advice and empowering patients to manage their health.
Support
frailty care
, repeat prescribing processes, and medicines reconciliation following hospital discharge.
Take forward
quality improvement projects and prescribing audits
that enhance safety and effectiveness.
Promote the implementation of
national guidelines and local formulary recommendations
.
Contribute to education, training, and public health campaigns.
If you are an Independent Prescriber or working towards this qualification, we are committed to supporting you to develop and maximise your prescribing skills.
What You'll Need
Registration with the
General Pharmaceutical Council
.
At least
2 years' post-qualification experience
.
A passion for improving patient care and a proactive, collaborative approach.
Excellent communication skills and confidence working in a multi-professional environment.
Knowledge of medicines optimisation in primary care.
Desirable:
Independent Prescriber qualification.
Experience working in General Practice.
