We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our ever growing PCN Pharmacy team of 12 pharmacists. The ideal candidate will play a crucial role in optimising patient care through effective medication management and collaboration with other healthcare professionals. This position requires a strong understanding of medication management, medicines reconciliation as well as good overview of quality medication reviews and medication optimisation. The Clinical Pharmacist will ensure that patients receive the highest standard of pharmaceutical care while contributing to their overall health outcomes.
Responsibilities
Review and interpret medicines management to ensure safe and effective therapy for patients.
Provide expert advice on medication, including dosage adjustments and potential drug interactions to patients and other healthcare professionals.
Collaborate with clinicians, nurses, and other healthcare professionals to develop comprehensive patient care plans.
Conduct medication reconciliation to prevent errors.
Monitor patient responses to medications and adjust treatment plans as necessary.
Educate patients and their families on proper medication use, potential side effects, and adherence strategies.
Participate in wider MDTs to provide pharmaceutical expertise in various care situations.
Maintain accurate records of medication therapies and interventions in accordance with regulatory standards.
Qualifications
A degree in Pharmacy
Registration with the General Pharmaceutical Council (GPhC) or equivalent body.
Proven experience in a clinical pharmacy setting, preferably within an General Practice/PCN environment.
Strong knowledge of pharmacology and LTC as it relates to patient care.
Excellent communication skills with the ability to work collaboratively within a multidisciplinary team.
Detail-oriented with strong organisational skills to manage multiple priorities effectively.
Commitment to continuous professional development and staying current with advancements in pharmacotherapy.
