We are seeking a highly skilled and dedicated Clinical Pharmacist to join our General Practice team. The ideal candidate will possess extensive experience in GP settings, with a strong background in managing complex medication therapies, patient assessment, and clinical pharmacy services. This role offers an opportunity to contribute to patient care within a dynamic healthcare environment, providing expert medication management and supporting clinical decision-making to optimize patient outcomes.
Responsibilities
Conduct
structured medication reviews (SMRs)
and optimize treatment for patients with long-term conditions.
Review
patients' conditions
,
hospital/clinic letters
,
medications
, and
prescriptions
to ensure safe, effective, and evidence-based prescribing.
Carry out
medication reconciliation
following hospital discharge or transfer of care.
Manage and process
repeat prescriptions
, resolving medication queries from patients and clinicians.
Support the achievement of
QOF (Quality and Outcomes Framework)
targets through medicines optimization and clinical audits.
Provide
clinical advice and guidance
to GPs and practice staff on safe and cost-effective prescribing.
Identify opportunities for
prescribing improvements
in line with local ICB and NHS guidelines.
Support
audit work
, data reviews, and reporting as required.
Undertake any
additional duties appropriate to the role
as requested by the GP partners or practice manager.
Qualifications
Registered Clinical Pharmacist
with the General Pharmaceutical Council (GPhC).
Demonstrable experience in
primary care or GP practice
settings.
Proficiency in EMIS Web
- essential*. Solid understanding of
QOF indicators
and primary care performance measures.
Proven ability to conduct medication and clinical reviews independently.
Excellent communication and teamwork skills.
Strong attention to detail and ability to prioritize effectively.
Desirable:
Independent Prescriber (IP)
qualification.
Experience in clinical audits and implementation of prescribing initiatives.
Familiarity with local
ICB prescribing policies
and NHS England frameworks.
