Merrow Park Surgery has an excellent opportunity for a

part time Clinical Pharmacist

to join our friend and patient focused practice in Merrow, Guilford. If you are looking for an opportunity to make a difference and can contribute to our Practice vision then perhaps this is the role for you.

This is a new role as we are looking to expand our services. The role will have regular contact with patients and will reduce the workload of GP prescribers in the practice by performing structured medication reviews. They will also support the management of patients with long-term conditions by reviewing repeat medication requests. The ideal candidate will play a vital role in ensuring optimal patient care through the safe and effective use of medications. You will collaborate with healthcare professionals to provide comprehensive pharmaceutical care, focusing on medication management and patient education.

We are looking for a Practitioner, who is enthusiastic and motivated to participate in a model of patient care. You will have fully accredited clinical skills qualifications, and have current registration.

Main duties of the job



Conduct thorough medication reviews and assessments for patients, ensuring appropriate therapy and adherence. Provide expert advice on medication administration and potential drug interactions. Collaborate with GPs and nursing staff to develop individualised medication plans for patients. Educate patients and their families about medications, including usage, side effects, and safety precautions. Maintain accurate records of medication therapies and patient responses. Stay updated on current pharmacological research and advancements to ensure best practices in patient care. Participate in our daily multidisciplinary team meetings to discuss patient care strategies.

Qualifications



A degree in Pharmacy from an accredited institution. Masters degree in Pharmacy or equivalent qualification Registration with the General Pharmaceutical Council (GPhC). Independent prescriber (not essential) Strong knowledge of anatomy and physiology as it relates to pharmacotherapy. Experience in a clinical setting, particularly within primary care. Excellent communication skills with a focus on patient care. Ability to work collaboratively within a multidisciplinary team. Strong analytical skills for assessing medication regimens and making recommendations.
