We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our healthcare team. The ideal candidate will play a vital role in ensuring optimal patient care through the safe and effective use of medications. You will collaborate with healthcare professionals to provide comprehensive pharmaceutical care, focusing on medication management and patient education.
Duties
Conduct thorough medication reviews and assessments for patients, ensuring appropriate therapy and adherence.
Provide expert advice on medication administration and potential drug interactions.
Collaborate with physicians and nursing staff to develop individualised medication plans for patients, particularly in the ICU setting.
Educate patients and their families about medications, including usage, side effects, and safety precautions.
Maintain accurate records of medication therapies and patient responses.
Stay updated on current pharmacological research and advancements to ensure best practices in patient care.
Participate in multidisciplinary team meetings to discuss patient care strategies.
Qualifications
A degree in Pharmacy from an accredited institution.
Registration with the General Pharmaceutical Council (GPhC).
Strong knowledge of anatomy and physiology as it relates to pharmacotherapy.
Experience in a clinical setting, particularly within an ICU environment, is highly desirable.
Excellent communication skills with a focus on patient care.
Ability to work collaboratively within a multidisciplinary team.
Strong analytical skills for assessing medication regimens and making recommendations.
