About the role
The Clinical Referral Coordinator will play a key role in the review and management of new ADHD referrals into the organisation. They will undertake triage of referrals to determine clinical priority, assess risk factors, gather additional information as required, and ensure that individuals are directed to the most appropriate pathway for assessment, support or signposting.
They will work as part of a multidisciplinary team regularly engaging with external stakeholders and inbound teams to contribute to timely care for patients whilst awaiting an ADHD assessment. At ADHD360 we use technology to make the patient journey as succinct as possible but due to expansion of the organisation we require additional support to offer dedicated clinical oversight to the process and the best possible patient experience.
Duties and Responsibilities:
Carry out comprehensive review and assessment of need for patients awaiting ADHD assessments ensuring that patients receive timely care according to clinical need
Assess patients' health needs and be able to assess and manage risk factors including safeguarding needs, using evidence based clinical knowledge
Providing advice, support and information to patients being referred to ADHD360
Deliver non-judgemental care that makes a positive difference to patient experience
Work alongside external stakeholders to manage risks for patients both under our care and awaiting assessment.
Offer clinical advice and guidance to inbound teams to support with urgent enquiries in relation to risk and safeguarding
Signpost new referrals to appropriate services and information relevant to the presenting complaint
Support informal training of administrative staff to improve patient experience
Work in conjunction with specialist advisors (Patient Safety Lead and Safeguarding Lead) to ensure emergent situations are actioned appropriately.
Escalate clinical concerns outside of the role through the appropriate escalation pathway.
Qualifications and Experience:
Essential
Qualified Social Worker with extensive Mental Health experience or Mental Health Nurse (registered with NMC/Social Work England/Social Care Wales)
work unsupervised and independently within the NMC Code of Professional Conduct/ BASW Code of Ethics
Educated to Level 6 or equivalent experience
Demonstratable mental health and risk management experience
Desirable
Demonstratable experience of telephone assessments
Experience working with people who have undiagnosed/ diagnosed ADHD
Experience working with safeguarding networks
Demonstratable Skills:
Excellent communication skills, ability to communicate effectively with staff and patients. Enabling complex technical information to be communicated in a style that colleagues can comprehend and implement.
Ability to analyse complex information and present information effectively to a range of audiences.
Knowledge of Local and National health agendas
Ability to build constructive relationships and networks with a wide range of stakeholders both internal and external.
Professional approach consistent with regulatory body expectations
Excellent time management and ability to manage own workload
Strong desire to uphold clinical standards, organisational values and deliver best practice
Ability to support and guide non-clinical colleagues to understand and engage with challenging conversations
Excellent navigation and liaison with external stakeholders in the delivery of person centred safe care
