We are looking for Associate Clinical Nurse specialists who can support our Integrated Community Services in Barnet and Enfield.



These posts offers a unique opportunity to get experience of working in specialist palliative care in the community. We are looking for enthusiastic, skilled nurses, who are patient focused and want to deliver the best care to patients and their family/friends. Ideally you will have experience in Palliative Care and are an excellent communicator who wants to make a difference.



The Associate CNS will contribute to the provision of specialist palliative and end of life care in the community for patients diagnosed with life limiting illness and their informal carers, in their own homes and in our primary care clinics as well as virtually and over the telephone.



You will work closely with the multidisciplinary team to provide advice, education and support to patients in their preferred place of care; and to other health and social care professionals. You will contribute and support the development and smooth running of the service.



You will work to agreed professional, departmental and Hospice boundaries as required to provide seamless patient centred care and take part in the provision of a 7-day specialist palliative care service.
