Job Description

UHSussex is pleased to have secured external funding for the role of Community Clinical Nurse Specialist - Stoma Care .



We are currently in the process of relocating Elective Colorectal & Lower GI Cancer Surgery and Stoma Reversal Surgery from Royal Sussex County in Brighton to the Worthing site, creating a centre of excellence for Colorectal Cancer Surgery delivered across at Worthing and St Richard's hospitals



This is an exciting development where we be investing in new theatre and bed capacity and associated surgeon, anaesthetic, nursing, therapies and other workforce requirements to meet the additional demand in Worthing.



This new post will be key in supporting the delivery of the service.



The post holder will work with the Stoma Care Nursing t e am in delivering and managing an expert Stoma Care Nursing Service for University Hospital Sussex NHS Foundation Trust predominantly in the community and at Worthing General Hospital. The role involve s patient centred individualised care to those undergoing Colorectal, Urology and G ynaecology surgery resulting in a stoma .



You will also support paediatric and neonatal stoma care services within both seco ndary and primary care settings . You will provide professional and clinical expertise to assist the physical, psychological , and social rehabilitation of patients requiring surgery involving stoma for mation .



You will provide specialist services and advice to patients and their families who potentially or require stoma forming surgery. This role will require the post holder to undertake community visits and will run nurse-led clinics and will teach a wide range of healthcare professionals with support from senior stoma care CNS's . The position would be suitable for a registered nurse with proven experience working as a Stoma C are CNS with Surgical and Colorectal experience . You will be look ing to further develop your career as a Sto ma Care Nurse specialist and will have a passion for the speciality a nd service development.



This post may be offered as band 7 but if not meeting the criteria , you will start at band 6 until all band 7 Stoma Care competencies are achieved.

Patient Care Delivery



Predominantly community based role but will be expected to work within Worthing hospital site as required, depending on workload or to cover leave. To accept referrals to see patients from consultant surgeons, ward staff and other members of the multidisciplinary team. To site for stoma formation in the optimum position to prevent physiological or psychological distress to the patient (this service should always be offered to elective patients and emergency patients wherever possible). To provide patients with advice on suitable appliances and accessories as required, always ensuring impartiality. Use clinical judgement and autonomous decision making concerning the interpretation and analysis of complex information from a variety of sources. Ability to analyse and interpret complex information, pre-empt, and evaluate issues and recommend an appropriate course of action. Liaise effectively with all members of the multidisciplinary team to formulate implement and evaluate an appropriate plan of care for the patient in a timely manner. To be responsible for own caseload of patients with new or established stomas within the hospital and provide continued advice and support on discharge. Plan and prioritise working day to best meet the needs of the service, whilst remaining flexible enough to respond to sudden changes in those needs. To contact and meet with patients preoperatively to provide information and education to them and their relatives/carers regarding their proposed surgery and stoma formation. To provide postoperative information, counselling, and support to ensure that the patients and their relatives/carers have a level of knowledge commensurate with their needs. To offer a supportive service applying specialist knowledge to enable patients to adapt to their surgery and stoma formation. To assist and advise ward teams with all aspects of care for patients with a stoma. To act as a clinical role model to others by working collaboratively with the ward teams in promoting best practice standards in stoma care. Promote safe and effective care of patients by having awareness of, and utilising Association of Stoma Care Nurses (ASCN) UK Clinical guidelines to underpin evidence-based practice. Ensure that any safeguarding issues are raised appropriately in accordance with Trust policy, to enable investigation in relation to vulnerable adults or children.
