An opportunity has arisen within South East Essex Dementia & Older Adults Community Mental Health Service for a Registered or newly qualified Band 5 Mental Health Nurse.
The successful candidate would be working with the Care Home Team which supports and assists in assessment of needs for individuals, supports diagnosis, appropriate on-going review and care planning for individuals in the residential care settings for those with a diagnosis of dementia or suspected dementia.
The post holder will work with a range of professionals and link with other services within South East Essex. This includes the Single Point of Referral (SPOR) and Southend University Hospital in particular.
The post hold will support people and their carer's in residential care settings to ensure high quality care, identification of deterioration or rising risk of admission, by providing timely intervention, support and specific training to carers.
The post holder will support the assessment of care needs and the development, implementation and evaluation of programmes of care for patients in Residential Care Settings.
The post holder will be expected to act as an ambassador for EPUT within their work setting. Undertaking assessments where required including planning, facilitating, implementing, and evaluating care for individuals with dementia to avoid unnecessary hospital re-admission and to provide ongoing support to the wards within the hospital.
Advise a range of health and social care professionals on effective treatment, interventions and management of individuals with dementia promoting independence.
Have a sound knowledge/understanding of the legislation as part of the role, including Mental Health Act 1983, Mental Capacity Act, NICE Dementia Guidelines and National Dementia Strategy.
To review at regular intervals with the clinical lead, the overall nature of the workload, work methods and techniques and evaluating clinical practice as a whole
Ensure that clients on the caseload with behavioural problems, e.g. violent or potentially violent behaviour are identified, and that other staff are aware of the risks involved, and that other staff act with caution and care is taken when visiting, if necessary, organising joint visits and remaining mindful of the lone worker policy.
