The Community Development Outreach Officer will develop close links with Local Authorities, Third Sector and community organisations to support communities to have a greater understanding of their rights and access to health care, as well as playing an important role in identifying trusted community intermediaries and establishing mechanisms to sustain future engagement.
The post holder will be expected to travel regularly to rural and urban areas to deliver a community outreach service across Pembrokeshire, Ceredigion and Carmarthenshire.
The post holder will work collaboratively with Health Board, Local Authority, community- based teams and other key stakeholders.
Main duties of the job
Develop strong links with trusted intermediaries and key groups and be able to identify key community influencers and establish on-going relationships to support continuous engagement activities.
Share priority public health and wellbeing messages, guided by Welsh Government, Public Health Wales and the Local Public Health Team.
Support Refugees and Asylum Seekers by working with colleagues, other public and third sector organisations, and utilising knowledge of the locality to mobilise others (volunteers, intermediaries etc.).
Support communities to be empowered and understand their rights and responsibilities in relation to keeping themselves safe and well.
Create opportunities to gather qualitative information related to the lived experiences of people experiencing poorer health outcomes in relation to health inequalities.
Become a bridge between the health messages and message givers (Government, Health Boards, Public Health Wales and others) establishing the Community Development Outreach Team as a trusted and reliable source of information for communities.
