Your Role:



As a key member of our team, you'll provide high-quality, proactive care tailored to the needs of your designated neighbourhood population. Utilizing your specialist skills in assessment, planning, and implementation, you'll deliver person-centred care that makes a positive impact.



Key Responsibilities:



Collaborate with Primary and Secondary care clinicians, hospital teams, voluntary organisations, and other professionals

Ensure the right care is delivered in the right place, by the right professionals

Share caseloads within the multidisciplinary team to maximise efficiency and patient outcomes

Demonstrate leadership, teamwork, delegation, and excellent care planning

Manage medication, health, and case coordination for individuals with complex needs



Ideal Candidate:



Proven expertise in care planning, assessment, and coordination

Strong leadership and teamworking skills

Ability to work collaboratively within a multidisciplinary team
