We are seeking a dedicated Mental Health Practitioner to join our forward-thinking Community Mental Health Service within Somerset NHS Foundation Trust. In this vital role, you will provide high-quality, evidence-based psychosocial interventions to individuals with severe and enduring mental health challenges. This is an exciting opportunity to work collaboratively across traditional boundaries of care, with a strong focus on a strengths-based, recovery-oriented approach.
You will receive regular supervision and mentorship, helping you to continue developing your clinical competence and confidence. You'll also have the opportunity to contribute to service development and provide leadership and support to other members of the team.
This role requires a valid UK driving licence and access to a car insured for business use, as the post involves regular travel throughout the locality to attend home visits and community meetings.
Main duties, tasks & skills required

Provide comprehensive assessments and evidence-based interventions for a caseload of service users experiencing a wide range of mental health difficulties.
Establish and maintain effective, therapeutic relationships with service users, carers and families.
Work collaboratively with a wide range of partners including GPs, social care, voluntary organisations and other mental health services.
Contribute to the clinical leadership of the team, offering supervision and guidance to junior staff as agreed with the Team Manager.
Identify and respond to safeguarding concerns and manage clinical risk in line with Trust protocols.
Create and communicate personalised recovery and risk management plans.
Participate in team meetings, service development initiatives, and clinical governance activities.
Maintain accurate clinical records using electronic systems (RiO).

You will be a Registered Mental Health Nurse (RMN) who demonstrates a passion for improving outcomes for people living with mental health difficulties in the community. You will work within a supportive multi-disciplinary team, delivering recovery-focused interventions rooted in compassion, empowerment and respect.
You will be expected to:
Apply your professional knowledge to assess mental health needs and deliver psychosocial interventions using approaches such as motivational interviewing and solution-focused therapy.
Support service users in managing crisis situations, preventing unnecessary hospital admissions and promoting recovery and independence.
Liaise effectively with colleagues and partner agencies to ensure coordinated, person-centred care.
Maintain responsibility for caseload management, prioritising and organising your work to meet the needs of service users.
Contribute to team training, audit, supervision, and quality improvement initiatives to ensure continued excellence in care delivery.
Use reflective practice and supervision to support your ongoing professional development.
