Are you a Registered Mental Health Nurse looking for a rewarding opportunity within crisis and urgent care services?




Our friendly and dynamic Home Treatment Team in Taunton is seeking skilled and compassionate Registered Mental Health Nurses for a 12-month fixed term contract to cover maternity leave. This is a fantastic opportunity to be part of a supportive, multidisciplinary team delivering high-quality, person-centred care in the community.


You'll work across both the Home Treatment and First Response Services, responding to individuals in acute mental health crisis and providing care and support that helps people remain at home and avoid unnecessary hospital admission.

This role will involve flexible working across a 24-hour shift rota (days and night shifts) in a 7 day a week service




Main duties, tasks & skills required

As a Senior Community Mental Health Nurse, you will play a key role within our Urgent Care Pathway, offering rapid response assessments and home treatment as an alternative to hospital admission.


You will be responsible for:

Holistic Assessments:

Conduct thorough psychosocial and risk assessments in a range of settings including clients' homes, acute hospitals and community venues.

Care Planning:

Develop and implement safe, creative and evidence-based care plans tailored to the individual's recovery needs.

Crisis Intervention

: Deliver short-term therapeutic interventions such as emotional skills work, behavioural activation and psychoeducation.

Medication Management:

Administer medication safely within the community and monitor for side effects.

Collaborative Working:

Work closely with families, carers and professionals across health and social care to ensure continuity of care.

Supervision and Support:

Provide leadership and supervision to Band 5 staff, students and support workers.

Service Development:

Contribute to quality improvement initiatives and the ongoing development of urgent care services in Somerset.

You will need excellent interpersonal and clinical skills, a flexible approach to shift work, and a commitment to providing compassionate, person-centred care.

Undertake urgent, holistic community-based assessments of individuals in acute mental health crisis. Deliver brief, recovery-focused interventions and coordinate care as a named keyworker. Act as a gatekeeper for inpatient beds and support early discharge planning from inpatient units. Participate in the First Response Service, offering timely triage and telephone support for those experiencing crisis. Work collaboratively within a multi-disciplinary team to deliver psychological, medical and social interventions. Manage risk effectively, including formulation, planning and escalation as needed. Promote and uphold the Trust's values of Kindness, Respect, Teamwork and Everyone Every Day in all aspects of care delivery.
