Community Nursing Team

The Hospice Community Nursing Teams provide palliative care support to adult patients and the people that are important to them at home.
Details:
Working hours: 37.5 hours per week, 9am to 5pm
Working days: Thursday - Sunday with an additional floating day, and flexibility to cover bank holidays
Although this role is being advertised as a full-time position, part time opportunities (30 hours per week Thursday - Sunday) would be considered
Permanent position
Office based at our Brentry site (BS10)
Daily tasks include -
Talking to patients, families, and health professionals on the phone
Completing outcomes from clinicians
Accepting and processing referrals into the service
Using EMIS Electronic Patient Record
Using Microsoft Office word, outlook and excel
Updating our rostering system
Applicants should be excellent communicators and must have the ability to deal efficiently with the general public, NHS colleagues and other health and social care providers. In addition, it is a necessity that applicants have a high standard of IT skills, have a positive approach to customer care and be able to work on their own initiative and as part of a team.
Applicants must be flexible and willing to cover bank holidays to cover any absence within the team.
