Overview:



This role is funded by the Priscilla Bacon Hospice Charity as part of a new collaborative approach with Norfolk Community Health & Care NHS Trust (NCHC) to enhance and support the needs of palliative patients and their families in their own home. The successful applicant will work directly with the NCHC Pathway Leads, in The Living Well Service to develop a team of volunteers and will be responsible for their recruitment, induction, training and ongoing support/management. Ensuring a positive experience for all volunteers and the patients and families we support.

During the pilot phase of the project, the Volunteer Co-ordinator will support the delivery and co-ordination of the community volunteer service, including the management and supervision of the volunteers, receiving referrals and matching the volunteers to the patients and their families/carers.

Main Duties & Responsibilities



Service Development



1. Support the Pathway Leads to review and research existing volunteer models locally and nationally.

2. Contribute to the Stakeholders Steering group which will inform the planning phase and project design.

3. Review patient and career feedback to determine the needs of the new service and how it can best be supported by a team of community-based volunteers.

4. Participate in the development and implementation of relevant policies and procedures in relation to the community-based volunteer service.

Recruitment and Selection



1. Work in collaboration with the Living Well Pathway Leads and other stakeholders such as Voluntary Norfolk to identify the tasks that could be undertaken by volunteers and develop role descriptions.

2. Liaise with the marketing and volunteering services to support the planning and implementation of an innovative and engaging volunteer recruitment campaign.

3. Develop community links and partnerships to ensure that our opportunities have a wide reach and take part in regular promotional events to aid recruitment and to ensure that there is a fair representation from all local communities.

4. Work with the Volunteering team to recruit, induct and support new volunteers.

Induction and Training



1. Plan induction and ongoing training to ensure that each volunteer has all the right skills and tools to provide the support needed in the community.

2. Support the Volunteering team and the Pathway Leads to develop appropriate mandatory training for newly appointed community volunteers.

Service Delivery



1. Work with the Place based Community teams to manage the workload of individual volunteers and co-ordinate the day-to-day activity.

2. Contribute to discussions with the Specialist Palliative Care Team and Community colleagues to inform the allocation of the volunteers.

3. Attend regular meetings in the community to identify patients who would meet the criteria for a Volunteer.

4. Carry out assessment visits on the phone or in referred people's homes and match to the volunteers accordingly and support the development of pathways for escalation.

5. Review the notes made by volunteers and monitor practice in collaboration with the Community teams and respond accordingly.

Volunteer Support and Supervision



1. Provide regular supervision opportunities for volunteers, through a range of face to face, telephone and group supervision sessions.

2. Work with the other volunteer coordinators to ensure that the volunteers are effectively managed and supported.

3. Gather feedback from the volunteers on their experiences of working in the service.

4. Respond to and help to resolve volunteering concerns and safeguarding issues, in line with PBHC policies and procedures.

Monitoring and Evaluation



1. Maintain up to date volunteer records using the charity's volunteer management system.

2. Provide reports on volunteer data and the impact of the new community service.

3. Use evaluation data from the project stakeholders to inform the project going forward.

Professional

1. Maintain accurate, contemporaneous records in keeping with NCH&C protocols and security policies.

2. Identify and request sufficient time to continue personal/professional development.

Organisational

1. To provide annual, monthly, weekly returns as required.

2. Manage diary in context of meeting the needs of the project and report and workload pressures accordingly.

3. Attend all mandatory training.

4. Participate in staff development and training as appropriate and use supervision to promote personal/professional development.

General

The post holder will be expected to participate in an annual appraisal of their work where the job description will be reviewed and objectives set. In line with the annual personal development plan the post holder will be expected to undertake any training or development required to fulfil their role.
