Job Purpose The aim of the Childrens 0-19 Public Health community nursing service is to reduce health inequalities and improve the health and wellbeing of children and their families in Nottingham City. The team offers a universal service for all children and young people in Nottingham as set out in the Healthy Child Programme. The role of the Community Public Health Nurse within the 0-19 Childrens Health Advice Hub To work within the 0-19 Childrens Health Advice Hub, supporting the Specialist Community Public Health Nurse (Health Visitor (HV) in the delivery of universal and targeted services. To support the HV and team in the management of the universal and universal plus transfer in caseload, as part of an 0-19 integrated multi-agency partnership approach to promote health and contribute to tackling health inequalities in line with local and national guidelines, public health priorities and Safeguarding responsibilities.
To support with the management of requests for advice and support from families and professionals via the telephone, along with triage of incoming referrals and families transferring into the area. To provide advice, signposting and support for parents, carers and young people using Text Health, our text messaging service. To work as part of a multi-disciplinary team, sometimes without direct supervision, but reporting back to the HV on a regular basis To work with a child & family focus, in a solution focused way. To promote holistic healthcare to the babies, children, young people, and families and to work in partnership with other agencies.
To identify when specific programs of care are required for babies, children and young people who have specific/complex needs and refer to the relevant 0-19 team for a follow up. To lead on and participate in health promotion initiatives e.g., childhood immunizations, in line with local and national guidelines, public health priorities and safeguarding responsibilities. To Support the development of the care pathways required for this population group to reach their optimum potential. To ensure that the rights, needs and dignities of babies, children, young people, and families are met by providing health promotion and support, referring appropriately to the relevant teams To promote the health of babies, children, young people to enable each child to reach his/her full development and educational potential which supports the philosophy of inclusive education.

Dimensions STAFF: You will be part of a Childrens Services team working together in an integrated way delivering the Healthy Child Programme to families and children aged 0-19 years BUDGET: A requirement to work in a cost-effective manner. AREA: Within Nottingham City boundaries Key Responsibilities Clinical Attend team / allocation meetings. Appropriately delegate work to skill mix team.
Provide education on local and national targeted public health initiatives via the telephone. Provide and receive routine information, orally, in writing or electronically in line with Nottingham CityCare policy and abiding by legal requirements. Communicate effectively with parents, carers, young people, and other professionals through different methods of communication, including telephone calls, our Text Health text messaging service, and emails. Ensure clear, accurate and up to date recording of information on SystmOne as per local record keeping policy.
To work in partnership with children, young people, and families and with local partners across the Nottingham Childrens Partnership, statutory and voluntary agencies To work with families identifying and taking appropriate action / make appropriate referrals in any situation where there may be vulnerability, safeguarding or health concerns. To access and participate in safeguarding supervision. To conduct oneself in a corporate and professional manner always maintaining confidentiality including compliance with the CityCare dress code policy. To attend essential, mandatory, and appropriate training courses and meetings, thus keeping professional development current.
Ensure practice is evidence based to maintain high quality, effective public health care. To demonstrate leadership and contribute to the appraisal, development, and monitoring of performance of team members. To maintain an appropriate learning environment to meet the educational and developmental needs of new starters, student nurses and other learners as required. To undertake mentorship, preceptorship, and clinical supervision as per policy.
