Evenings, weekends and bank holidays on a set rota.
1 x 30 hours per week post

About the role
Our Community Responder Volunteers Service, in partnership with Northamptonshire Healthcare Foundation Trust and the Integrated Care Board, supports vulnerable people throughout Northamptonshire. Working with the Intermediate Care Team (ICT) and Remote Monitoring Hubs to provide low level support to people in their own homes to prevent unplanned hospital admissions.
As Community Responder Triage Coordinator, you will be based at Lotus House in Northampton working alongside Health Care Teams. You will be responsible for triaging live referrals and coordinating volunteers and staff to attend visits to someone's home when they are in need.
We can visit to check on someone's welfare
We support the Intermediate Care Team to assist people who have fallen
We collect urgent prescriptions and deliver to the patient
We collect and deliver low-level mobility equipment
We can stay with someone for up to 2 hours to provide reassurance and comfort when there is an anticipated delay in urgent care from health and social care
Our Coordinators will contact the person after the initial visit has been completed to explore any longer-term support needs. They can link people with longer-term support services such as Collaborative Care Team, Ageing Well and other partner organisations.
You will promote the work of Community Responders and Age UK Northamptonshire throughout Northamptonshire and will be responsible for developing and maintaining good working relationships with health professionals and other service providers.
You will be responsible for ensuring all patient identifiable information is stored and disposed of appropriately, and that Information Governance and Confidentiality procedures are adhered to.
You will work pro-actively and in a task centred way. You will need to be able to absorb information and plan ahead, working in a systematic and organised way. You will need to be able to think quickly on your feet and be able to use your experience an initiate to decide a best course of action in challenging times. You will be motivated by challenge and enjoy seeing problems through to their solution, knowing that your timely actions have improved life for someone else.
Good knowledge of community support services is desirable, alongside experience of care work or a professional qualification in Health or Social Care. If you thrive on being flexible and meeting new people, we would love to hear from you.
The role may require you to complete community visits therefore you must be a car driver with access to your own vehicle.
