An exciting opportunity has arisen for a Physiotherapist to join our Community band 5 rotation. The post will include rotating into Planned Community Therapy, Community Integrated Neuro and Stroke service (CINSS) and our new and transformational One Coventry Integrated Team (OCIT).
This role will enable you to develop knowledge and skills that support the future of community based rehabilitation.
You will play a key role in admission prevention, hospital discharge, proactive and preventative care through rehabilitation within the community .
Our team of specialist therapists strive towards facilitating independence, working closely with GPs, nursing services, social care to enable an integrated care approach. You will work closely alongside them to assess patients effectively, to understand what matters most to our patients gaining skills to develop clinically reasoned intervention that make a positive difference to the lives of the people in the community.

A full UK driving license is essential for this role. Please note that an international driving license is not accepted.
Successful applicants will be required to provide an immunisation and vaccination report from either their current UK occupational health provider or their UK GP.
This opportunity will enable you to be part of an innovative therapy team where teamwork and collaborative working are the cornerstone of effective patient centred care and staff development.
You will be offered a comprehensive induction to post, a robust in-service training /peer support programme and clinical supervision from experienced and specialist Physiotherapists.
You will assess patients in their own homes, in temporary or permanent residential or nursing care to maximise and empower rehabilitation potential, return to baseline, supporting self-management and adjustment to living life differently.
You will develop skills to;
Work as an effective team member
Complete effective assessments of therapeutic need
Establish rehabilitation prognosis.
Set appropriate person-centred goals.
Create collaborative plans enabling self-management, supporting a proactive and preventative agenda.
Patient care and therapeutic intervention is supported by our unregistered workforce of dedicated Assistant Practitioners and Technical Instructors. You will play a lead role in supporting their development.
