This is an exciting opportunity for a motivated professional to expand their skills in Home Treatment, Psychiatric Triage, and Psychosis care. You'll play a key role in coordinating care, leading multidisciplinary meetings, and supporting service users on their recovery journey. With a focus on collaboration, inclusion, and empowerment, you'll help individuals take ownership of their care while working closely with families, carers, and partner services to ensure the best possible outcomes.

What you can add:

You will independently carry out assessments, care planning, and care coordination, using strong interpersonal skills to listen, respect, and value individuals from diverse backgrounds. You will lead regular meetings with community team members to discuss, report on, and evaluate client care. You will encourage and empower service users to take responsibility for their care, engaging families and carers where appropriate, in line with recovery-focused principles. You will support service users to adopt recovery strategies that promote wellbeing, independence, self-care, and a healthy lifestyle. You will engage in reflective practice through supervision and peer groups, and contribute to the clinical supervision of junior staff.
What you'll need:

Qualified Social worker with current professional registration Experience of Home Treatment or Psychiatric Triage Experience of working with people who have Psychosis Experience of working in a Community Mental Health Team
System Knowledge: EPJS - mandatory
