Role Introduction
A fantastic opportunity has arisen for enthusiastic and dynamic Staff Nurses to join our Integrated Neighbourhood Teams. We currently have vacancies at both Seacroft and Chapeltown Neighbourhood Teams.
We have bases in Wetherby, Chapeltown, Seacroft and Meanwood, providing care in patients homes, whilst enabling early discharge from hospital and reducing hospital admissions.

We have split our ways of working into planned care and unplanned care, i.e. call outs and urgent assessments and there is the opportunity to work solely in the unplanned response service or both. In the unplanned Response Service, you will be working more closely with our Frailty Ward Matrons.
The service runs 7am to 10pm every day with various shift patterns, and we can offer flexible work opportunities.

You will work alongside District Nurses, Community Matrons, Occupational and Physiotherapists, Neighbourhood Clinical Assistants, Therapy Assistant Practitioners and Self-Management Facilitators in one MDT to provide truly holistic care for our patients.
You will work closely with colleagues in our Citywide Service such as the Falls, Continence, Urology and Colorectal Care and Palliative and End of Life Care teams and GP services to co-ordinate the best care for patients.
You will have great communication skills and maintain a high standard of clinical work when carrying out nursing procedures.
You will provide person centred care, helping patients to reach their goals and promoting self management.
You will be resilient, motivated, and adaptable to working in an ever-changing environment and of course, a passion for providing the best possible care.
You will ensure compliance with our clinical governance framework, you will motivate colleagues, champion service user safety and dignity and drive continuous improvement.
