This role is within of the Urgent Care Services West Kent which provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare. This may include treating acutely unwell people that would otherwise require hospital admission, and also supports hospital discharges. The service convers 24 hours a day 365 days a year.
Nurses use a wide clinical skills base to provide hospital-level care at home. This includes IV therapy, end of life symptom control and management via syringe driver, catheterisation skills, phlebotomy, wound management, risk assessment, as well as general observations and fundamental nursing care.
This service is providing nursing care within patients' homes and covering West Kent so the ability to drive is essential to this role.
