An opportunity has arisen for a dynamic and change focused, Community Team Manager to join the Central Norfolk Older Person Community Teams, based at Sophie Centre, Julian Hospital. Norwich.
This role includes providing clinical and operational leadership to the Team, including line management responsibilities, and ensuring effective governance processes. You will be responsible for ensuring effective service delivery and driving development of our service at team level.
You will be expected to p rovide visible leadership, promoting a values based culture and acting as a role model within the environment while delivering aligned to our local priorities.
You will be r esponsible for leading and managing the team, ensuring they are suitably trained, informed and competent to carry out the full range of their duties, and that mechanisms are in place for regular and effective supervision, appraisal, objective setting and personal development planning.
Demonstrat ing a clear understanding within the service of service user needs and care requirements. Contribute to MDT decisions regarding care based on knowledge and expertise.
Ensure care is delivered in a patient centred framework involving service users, families and carers. Ensure that service user and carer feedback contributes to the development and improvement of the service.
Ensure that the team practices within the trusts policies, protocols and standards, including NICE guidance where possible, including the reporting of complaints and incidents.
Ensure effective systems are in place to embed learning and continuous improvements from incidents and complaints.
Ensure that tasks and responsibilities are effectively, fairly and evenly distributed within the team to provide effective and responsive care for service users and carers
