About the role:
The role will involve working with people who approach the local authority, PtHB and other partners for assistance in relation to potential homelessness, who have multiple complex needs, including substance misuse, mental ill health. Persons presenting will often lead chaotic lifestyles, requiring the post holder to ensure all statutory agencies and others engage to provide appropriate assistance, and where this is not happening, call multi-disciplinary meetings to resolve.

Ability to work under pressure, to targets, manage and prioritise a number of competing demands
Excellent interpersonal skills and communication skills including good letter writing and verbal communication skills
Ability to relate to and gain the confidence, trust and respect of key internal and external partners
What you will do:
-Collaborate with Housing, Mental Health, and Substance Misuse teams to assess individuals and identify suitable, sustainable housing and support options.
-Support individuals with complex needs--primarily mental health and substance misuse--who are homeless or at risk, helping them maintain or secure accommodation, including temporary housing.
-Work closely with internal teams and external agencies (e.g., Housing, Health Board, voluntary/statutory services) to address homelessness and improve holistic support options in Powys.
-Act as the lead for complex needs assessments, advising on support plans for street homeless individuals and those with mental health/substance misuse issues, including appropriate referrals.
-Manage a caseload of complex needs clients, supporting them in maintaining accommodation and complying with tenancy/licence conditions.
-Assist Service Users in developing Personal Wellbeing and Housing Plans (e.g., Care and Treatment, Substance Misuse, Advanced Directives), ensuring data protection and alignment with personal goals.

Pay: 33,366.00-35,235.00 per year
